About the Program Course Content
Module 1: Initial License Application Process
- Prerequisite Certifications: Detail the requirement for passing the American Board of Opticianry (ABO) exam for the SLD license and the National Contact Lens Examiners (NCLE) exam for the CLD license.
- Application Submission:
- Walk through completing the required application form (Form 41-4, if applicable, or the most current form).
- Explain associated fees and payment methods.
- Review documentation requirements, such as transcripts, proof of experience, and exam scores.
- Fingerprinting and Background Check:
- Provide instructions for the mandatory Live Scan fingerprinting process for California residents.
- Explain the procedure for out-of-state applicants using fingerprint cards (FD-258).
Instructor qualifications
Upon completion of the course participants will be knowledgable on applying for a license.
Course Objectives
By the end of this course, the participant will be able to:
- Objective 1: Execute Application Filing
Successfully complete a licensure application via the DCA BreEZe Online Portal by correctly applying for a SLD and/or CLD License - Learn to renew license every 2-years

Learning Objectives
Course is provided once per month on the second Tuesday of the month by virtual meeting. Attendees will be required to record signature on a sign-in sheet.
